The application is to provide the DCFS employees and Health and Safety coordinators a convenient way to apply and process health and safety related requests, such as FMLA, IA, ADA, and more.

DCFS employees can log in using DCFS email address and Internet password. Note that the application is restricted to DCFS employees only and can be accessible from DCFS intranet and public internet.

• Login to the Health and Safety application using https://healthandsafety.dcfs.lacounty.gov
• Click on Login from opened Login page
• Enter/select your DCFS email address
• Enter internet password
• Following MS security steps
• User home page (based on user role) will be opened
  • Select a request type i.e. Special Equipment/ADA
  • Complete the open questioner page
  • Upload requested file if there is any
  • Click on Submit

To retrieve a forgotten password, you may click on the “Forgot Password?” link on the Login page. Page is directed to ISD password reset page.

A locked account is an account that tried to login for several times using wrong password.

For security purposes, your account will be locked after 5 failed logon attempts. Please click on the Los Angeles County Self-Service site link from the Forgot Password Window to unlock your account.

Upon submitting H&S request, system will assign to Return to Work (RTW) coordinator based on your pay location, and send you an email with the request number, and name of assigned RTW.

On the Home page, you will see a list of request(s) you submitted. Click on the request number hyperlink to view or print a request as needed. Once a request is submitted, you will not be able to make any changes to the report, but you can upload file if it is requested by RTW.
